required for application to PUMS Poznan
- in English unless otherwise stated:
1) Completed and signed application form.2) Medical Health Certificate, correct version:
Medical Health Certificate, MD, DDS and Pharm.D. version (including chest X-ray report), completed by a doctor.
Medical Health Certificate, Physiotherapy version (including chest X-ray report), completed by a doctor.
3) A copy of vaccination records, showing if Hep. B and TB shots were taken.
4) High school diploma (vitnemål/slutbetyg) and, if applicable, college diploma(s) - original or a copy certified by a Notary Public (Notarius Publicus) or a Polish Consulate. We need the diplomas in the original language and in English version (original or certified by a Notary Public or a Polish Consulate).
5) If you complete high school or relevant courses at secondary or higher level this year: Official high school transcript - an original or a copy certified by a Notary Public or by a Polish Consulate, with a certified translation into English (original or certified by a Notary Public or a Polish Consulate).
6) A photocopy of valid passport certified by a Notary Public or by Agder Vitenskapsakademi.
7) 6 recent passport-size photographs signed on the back (no head cover).
8) Two letters of recommendation on school letterhead: One from a science faculty member and another one from guidance teacher, counselor, principal or teacher of another subject.

9) Personal comments / motivation letter
We recommend that you write a short letter about your background and motivation for the program that you apply to, why you want to study at PUMS Poznan and how you have prepared yourself to live and study in Poznan/Poland for a number of years. This may be expressed in your application form (VIII,8 Personal Comments) or in a separate and signed document.
10) Job certificates
We recommend that you also send certificates documenting your most important/relevant work experience. These should be written in English.
Additional documents
The documents below are not required to receive the acceptance letter. They are to be submitted prior to the start of classes, as they are a prerequisite for receiving student ID in Poznan:
1) Proof of health insurance valid in Poland (EHIC for European citizens)
2) A copy of student visa or other document confirming that the stay in Poland is legal.
