Collegium Medicum, NCU, Bydgoszcz

Documents required for application to Collegium Medicum

1) Completed and signed application form.

2) Medical Health Certificate, completed and signed by a doctor.

3) A copy of vaccination records.

4) High school diploma (vitnemål/slutbetyg) and, if applicable, college diploma(s) - original or a copy certified by a Notary Public (Notarius Publicus) or a Polish Consulate, and with an Apostille. We need the diplomas in the original language and also the English version (original or a copy certified by a Notary Public, and with an Apostille).

5) If you complete high school this year, please send an official high school transcript from your school, with your subjects and grades from the first and second years of school, and the first term of the last year.
This should be in original, or otherwise a copy certified by a Notary Public, and with an Apostille. You should also include a certified translation into English, in original or a copy certified by a Notary Public, and with an Apostille.

6) A certified photocopy of valid passport.

7) 4 recent passport-size photographs signed on the back (no head cover).

8) Letter of recommendation from high school teacher(s).

9) Personal comments / motivation letter
We recommend that you write a short letter about your background and experience and your motivation for the medical programs. Why do you want to study at Collegium Medicum and in what ways are you prepared to live and study in Bydgoszcz/Poland for six years. This may be expressed in your application form (V, 5 Personal Comments) or in a separate and signed document.

10) Job certificates
We recommend that you also send certificates documenting your most important/relevant work experience, especially if you have worked in the health sector. These should be written in English.
